Guiding Your Transition to Adult Healthcare

Updated October 13, 2023

At Dr. Bou Pediatrics, we are dedicated to providing comprehensive and compassionate healthcare from infancy through adolescence. As our patients approach adulthood, their healthcare needs evolve, necessitating a transition to adult medical care. Here’s what you need to know as you or your child approaches the 18th birthday:

1. Document Assistance:

  • Our team is here to assist with any required college entry or military forms, ensuring a smooth transition in this aspect of life.

2. Medication Refills:

  • It’s crucial to initiate the process of finding a new healthcare provider before the 18th birthday, especially if there are active medication refills.
  • In certain cases, one additional refill may be approved post the 18th birthday, subject to executive approval.

3. Special Circumstances:

  • Exceptionally, in cases involving complex medical care, an executive may approve continued medical care beyond the 18th birthday.
  • Each case will be individually evaluated to ensure a smooth and beneficial continuity of care.

4. Finding a New Doctor:

  • Utilize the tools provided by insurance companies to locate a new practice, preferably with an Internal Medicine doctor for adult medical care.
  • We will coordinate the transfer of medical records to ensure a seamless continuity of care, once a signed medical records release form is provided.

5. Next Steps:

  • Contact the insurance company to request a reassignment to a new doctor.
  • Obtain a medical records release form from the new doctor, complete it, and return it to us to facilitate the transfer of medical records.

Our team is committed to supporting a smooth transition during this important phase. Should you have any questions or need further assistance, please feel free to contact our office.

We are immensely grateful for the trust and confidence our community has placed in us over the years. While transitions come with mixed emotions, we are confident it's a step towards ensuring age-appropriate medical care as our patients step into adulthood.

Thank you for being a vital part of the Dr. Bou Pediatrics family. We wish everyone the very best in their journey towards continued good health and well-being.
