School and sports physicals
We complete Florida school entry physicals and FHSAA sports physicals (form EL2) at both offices, usually within the same week. If your child needs vaccines for school, we give them at the same visit - one trip, done.

Do you do Florida school and sports physicals?
Yes. We complete Florida school entry physicals and the FHSAA sports physical (form EL2) at both our Wesley Chapel and Plant City offices. Book online or call (813) 948-8814 - same-week spots are usually available, and we add extra room before school starts.
What should we bring to a school physical?
Your child's school or league form if they gave you one (or download it below), any glasses or inhalers your child uses, and your insurance card. You do not need to bring shot records - if your child got their shots in Florida, we can pull the state record ourselves. New to our practice? Bringing your child's medical records from their last doctor helps the most.
Is a school physical the same as a well visit?
They overlap but are not the same. A yearly well visit is a full head-to-toe checkup that most insurance covers in full. We complete school and sports forms as part of a current well visit, so if your child is due, we do both in one appointment.
Want to save a few minutes? Bring your school or league's form (like the FHSAA EL2) printed and filled out with your child's information - we complete and sign the medical part at the visit.
